AL BUKHARI CENTER INC.
GENERAL PARTICIPATION CONSENT, LIABILITY WAIVER, MEDICAL AUTHORIZATION & MEDIA RELEASE
This Agreement applies to all programs, schools, classes, workshops, seminars, educational programs, religious programs, youth programs, adult programs, sports, fitness activities, recreational activities, childcare, camps, tutoring, counseling, volunteer activities, special events, field trips, transportation, and any other activity, service, event, or program conducted, sponsored, organized, or hosted by Al Bukhari Center Inc., whether on or off its premises.
1. PARTICIPATION CONSENT
I, the undersigned participant or parent/legal guardian of the participant named above, give permission for myself and/or my child to participate in all activities, programs, classes, workshops, services, events, recreational activities, educational activities, religious activities, field trips, transportation, and any future activities offered or sponsored by Al Bukhari Center Inc. I understand that participation in these activities involves inherent risks that cannot be completely eliminated. Parents or participants are financially responsible for any intentional or malicious damage caused to the facilities, equipment, or property of Al Bukhari Center Inc.
2. FIELD TRIPS & TRANSPORTATION CONSENT
I authorize participation in all scheduled field trips, off-site activities, and community events sponsored by Al Bukhari Center Inc. I further authorize Al Bukhari Center Inc., its employees, staff members, volunteers, and authorized representatives to transport the participant by school bus, charter bus, passenger van, private vehicle operated by authorized personnel, rideshare, public transportation, or any other authorized means of transportation.
I understand that reasonable supervision will be provided during transportation and off-site activities.
3. PHYSICAL ACTIVITY CONSENT
I understand that participation may involve running, jumping, climbing, sports, fitness activities, recreational games, inflatables, playground equipment, physical education, water activities, educational activities, outdoor activities, religious activities, workshops, and other physical or recreational activities.
I certify that I or my child is physically capable of participating unless written medical restrictions have been provided. I agree that it is my responsibility to notify Al Bukhari Center Inc. of any medical conditions, allergies, disabilities, medications, behavioral concerns, dietary restrictions, or physical limitations before participation.
4. MEDICAL AUTHORIZATION
In the event I cannot be reached during an emergency, I authorize Al Bukhari Center Inc., its employees, volunteers, and representatives to obtain emergency medical treatment for myself or my child when deemed reasonably necessary. I understand that every reasonable effort will be made to contact me before treatment whenever possible. I accept full responsibility for all medical expenses incurred.
5. PHOTOGRAPHY & MEDIA RELEASE
I grant permission to Al Bukhari Center Inc. to photograph, videotape, livestream, audio record, or otherwise capture images or recordings of myself or my child while participating in any activity. I authorize these photographs, videos, recordings, and likenesses to be used without compensation for social media, websites, promotional materials, brochures, newsletters, advertisements, educational materials, fundraising materials, printed publications, television, digital media, and future promotional purposes. I understand these materials become the property of Al Bukhari Center Inc.
6. ASSUMPTION OF RISK
I understand that participation in any activity sponsored by Al Bukhari Center Inc. involves inherent risks, including but not limited to slips, trips, falls, collisions, sports injuries, playground injuries, recreational injuries, equipment failure, physical exertion, heat-related illness, dehydration, allergic reactions, insect bites, food-related reactions, transportation incidents, weather conditions, communicable illnesses, injuries caused by other participants, property damage, and other known or unforeseen accidents. I knowingly and voluntarily assume all risks associated with participation and the use of Al Bukhari Center Inc.'s facilities.
7. RELEASE OF LIABILITY
To the fullest extent permitted by applicable law, I, on behalf of myself, my child, my heirs, executors, administrators, personal representatives, successors, and assigns, release, waive, discharge, and covenant not to sue Al Bukhari Center Inc., its board members, directors, officers, employees, volunteers, teachers, instructors, counselors, coaches, assistants, agents, contractors, sponsors, affiliates, representatives, and property owners from any and all claims, liabilities, demands, causes of action, damages, losses, costs, expenses, or attorney's fees arising out of or relating to participation in any program, activity, class, workshop, event, transportation, field trip, recreational activity, educational program, religious program, or use of the facilities of Al Bukhari Center Inc., including claims based on ordinary negligence, to the fullest extent permitted by law. This release does not apply to claims arising from gross negligence, reckless conduct, intentional misconduct, or any liability that cannot legally be waived under applicable law.
8. INDEMNIFICATION
I agree to defend, indemnify, and hold harmless Al Bukhari Center Inc., its board members, officers, directors, employees, volunteers, agents, representatives, contractors, affiliates, and sponsors from and against any claims, damages, liabilities, judgments, costs, expenses, or reasonable attorney's fees arising out of my or my child's participation in any activity, violation of this Agreement, or negligent or intentional acts.
9. RULES OF CONDUCT
I understand that participants are expected to follow all rules, policies, safety procedures, and staff instructions at all times, respect staff, volunteers, participants, visitors, and property, refrain from bullying, fighting, harassment, abusive language, vandalism, possession of prohibited items, or disruptive behavior, and conduct themselves in a safe and respectful manner. I understand that failure to comply may result in suspension, dismissal, removal from activities or the facility, or other disciplinary action without refund when applicable.
10. ACKNOWLEDGMENT
I certify that I am the participant or the parent or legal guardian of the participant named above. I have carefully read this entire Agreement, understand its contents, have had the opportunity to ask questions, understand that I am giving up certain legal rights as permitted by law, and voluntarily agree to all of its terms. I understand that this Agreement is intended to be legally binding upon myself, my child, my heirs, personal representatives, successors, and assigns
